Hebrew Hospital Home, Inc. has a fine history in geriatric long term care and in providing the highest quality care and services to its patients, participants and residents.
The Hebrew Hospital Home Inc. and its Adult Day Health Care Program of Westchester has developed a Social Model Adult Day Program for residents of the Westchester community. This Social Model program is specifically designed for persons who suffer from functional medical disorders, cognitive deficits, and/or mild to moderate Alzheimer’s disease and related dementias. The primary goal of this program will be to keep individuals in the community for as long as possible while increasing confidence and feelings of self-worth. Further, care giver respite is a strong component of this program.
This Social Model Adult Day Services Program currently operates three days a week for six hours a day with an emphasis on engagement through activities/creative arts, case management and some personal care. Future plans are to increase operations of this program to five days per week as the demand increases.
The Social Model program offers supervision, socialization, group and individual therapeutic activities and nutritional services including two meals per day.
Services can include:
- Physical, Social, Cognitive and Creative activities designed to limit disabilities and maximize strengths
- Art and Music therapy
- Exercise and Wellness
Nutrition by a Registered Dietitian
- Nutritious family style meals
- Dietary monitoring and counseling
- Nutrition education
Case Management by a Licensed MSW
- Individual/Family/Group Counseling
- Information and Referrals
- Supervised, door to door transportation to and from program, if needed
